Date Immigration Status Granted for 5 year bar not captured/used in application & CoC script, mapping, evidence and rules

Need to modify our script to capture additional details to ensure that we correctly determine the 5 year bar period in our rules.

1. Script Changes - All scripts - Citizen, case worker, CoC
We are proposing the following changes to our script
When an applicant indicates that they are Lawfully Present and enters a date of entry, additional conditional questions should be displayed to capture The Date Immigration Status granted.

2. Modify the Citizen Status entity
In addition to updating the script we will need to modify our Citizen Status entity and create two new attributes to store the Date of Entry, and the Date Status granted.

3. Modify the datastore to evidence mapping.
map Date of Entry from script to the new Date of entry attribute on the entity
map Date Immigration Status Granted to Date Status granted attribute on the entity
( I am not sure what date is currently used as the start date on this entity, the date of entry is currently probably mapped to the start date on the entity, don't think it is a good idea to do this, now as it is not very clear to the user, I think we need to have explicit labels on these two dates as they are important for the determination of the 5 year bar.

4. Modify the Non Financial Citizenship Rules in both Portal and Case Management
Need to modify the Non Financial Citizenship rules to ensure that we use the "Date the status was granted" as the date from which to determine if the person has met the 5 year bar. Currently we use the date of Entry which is not correct.
